Welcome to the MyMonHealth patient portal, a private and secure online management tool, providing easy online access to part of your medical record.  This access may also be granted to a parent or child of a patient.

Look for lab results, radiology reports,discharge documents and patient instructions from your visits. Other information in the patient’s electronic medical record is available, such as medications, allergies, immunizations, health issues and Advance Directives.

MyMonHealth is HIPAA-compliant, and your personal health information is protected.

How to get a MyMonHealth account

To get a MyMonHealth account, you can self-enroll at the following website https://mymongen.iqhealth.com/self-enroll/ or elect to have a portal account created at any registration location at a Mon Health Medical Center facility. Your legal name is required to self-enroll. An email address is also a requirement to start and to help manage your account information.

You will receive an email confirmation welcoming you to MyMonHealth. Within the email message will be a link to claim your account. Follow the link and enter the required security information.

If you want a trusted family member or friend to have access to your information, or if you want to have access to your child’s information, an account can be set up for that individual. If you are signing up for a child, you will need to provide a photo ID at the Registration Department and fill out a Proxy Request Form. If you want a family member or friend to have access to your information, you both will be required to provide a photo ID and sign a Proxy Request Form.

What do your results mean? If you have received lab work, we encourage you to contact your primary care physician to discuss the results.

What results are on the Patient Portal?

  • All laboratory test results
  • Radiology summary report - text only

What results are NOT on the Patient Portal?

  • Xray or Ultrasound images
  • Charts & graphs from testing procedures
  • Sleep study or EKG
