Requests for copies of Medical Records

Mon General is committed to protecting the privacy of your medical information.  
The following is a step-by-step guide for obtaining medical records from Mon General Hospital, Mon EMS and the Wound Healing Center.

To obtain records from a physician office, please contact that office.

To request medical records from Mon General Hospital, Mon EMS, the Wound Healing Center or to request CDs of Radiology or Cath Lab images, you must complete and sign an Authorization for Release of Protected Health Information form.   This form tells us which records to release and where you want them to go, such as to an individual, a physician, medical practice, facility or an agency. 

You may obtain this form at our office, located in the Mon General Hospital Main Lobby, in the Registration area.  You may also deliver the completed form to us at this location.  But please understand that there is NO guarantee that we can provide your medical records while you wait.

Cost for Medical Records

The cost of medical records depends on the format of the records, and to whom they are sent or given.

Records sent to a physician, medical practice, facility or agency
No Charge
Records mailed or personally handed to an individual
$0.36 per page, plus postage if mailed.  (Hard copy only.  We are unable to provide digital text files at this time.)
Radiology or Cath Lab CDs sent to a physician, medical practice, facility or agency
No Charge
Radiology or Cath Lab CDs mailed or personally handed to an individual
$5.00 per CD, plus postage if mailed

If you choose to personally pick up your medical records at our office, please be aware that we may NOT be able to provide them to you while you wait.

Mon General Hospital and Mon Health System employees are not exempt from fees.



MyMonGen patient portal

As a patient of Mon General, MyMonGen allows you online access to a portion of your medical records at no charge. This access may also be granted to a parent or child of a patient. MyMonGen is a private, secure online management tool that provides you with easy access to selected portions of your Mon General health information and to some of our hospital-based care providers.  Learn More about MyMonGen.


Medical Record Amendment

Every Mon General patient has the opportunity to request an amendment to his or her health information. Please complete the Request for Amendment of the Medical Record and mail it to the HIM Department at the address listed above. Please understand that your physician may be consulted and Mon General has the option of denying the request if your record is deemed accurate and complete. This process may take up to 60 days from the date the amendment request is received.
