Medical Records are obtained by appointment only.
We are closed on holidays.
Please allow 3-5 business days for completion of a medical records request. Please note that It can take up to 30 days as allowed by law. Per Mon Health policy, adult medical records are retained 10 years, birth/minor records for 21 years.
Continuing Care releases are given priority. To be considered a Continuing Care release, select "Other" on the form and write in your physician office's address as the recipient - do not select "Me" on the form. To help us meet your needs, please also indicate that you have an upcoming appointment as the "Purpose" including the date of your appointment.
MyMonHealth patient portal
As a patient of Mon Health Medical Center, MyMonHealth allows you online access to a portion of your medical records at no charge. This access may also be granted to a parent or child of a patient. MyMonHealth is a private, secure online management tool that provides you with easy access to selected portions of your Mon Health Medical Center health information and to some of our hospital-based care providers. Cost for Medical Records
The cost of medical records depends on the format of the records, and to whom they are sent or given. Below are the standard fees for producing a copy of your medical records.
- Search and handling fee: $20
- Electronic copies: $0.20 cents per page
- Paper copies: $0.40 cents per page
- Images on CD: $10
- Sales tax: 6%
Charges will not exceed $150 inclusive of all fees, including a search and handling fee, except for applicable taxes based on WV code. There is no charge for Continuing Care requests if records are sent directly to a medical provider or medical facility.
Medical Record Amendment
Every Mon Health Medical Center patient has the opportunity to request an amendment to his or her health information. Please complete the Request for Amendment of the Medical Record and mail it to Mon Health Medical Center, Attn: HIM Director, 1200 JD Anderson Drive, Morgantown, WV 26505. Please understand that your physician may be consulted and Mon Health Medical Center has the option of denying the request if your record is deemed accurate and complete. This process may take up to 60 days from the date the amendment request is received.
